Prevention in Primary Healthcare Project 2013 The Heart Foundation is working with your Medicare Local to build capacity in primary health care settings to refer adult patients who have been identified with a lifestyle related risk factor for chronic disease to services which will support behaviour change. The lifestyle factors of interest for this Project are the SNAPO risk factors: Smoking, poor Nutrition, Alcohol, Physical inactivity and Obesity.
The initial phase of the project involved working with a small number of General Practices and Medicare Locals in NSW to increase referrals between general practice and the wider primary care setting, including government and community services, and private allied health professionals. What was developed in collaboration with those practices and Medicare Locals has been incorporated into a new program of activity for 2013 which will look at identifying and providing support for the networks of services which provide lifestyle risk factor reduction activities.
In this next phase of the project we will be working more closely with referral services to better understand the networks that currently exist between these services and other primary health care settings. To help map the networks at a local level we will be undertaking a Network Analysis. We have contacted you specifically as your Medicare Local has suggested you as one of the services to which GPs refer patients with lifestyle risk factors.
The network analysis involves completing a short online survey about services you currently work with and the barriers and potential enablers to the referral process. The network analysis will help to identify:
All survey data will be collected electronically by an external company (Synergia) and will be de-identified to organisational level for the purpose of analysis and reporting.
We will be providing you with a written report on the findings as well as an opportunity to attend a seminar to further discuss the outcome of the Network Analysis and potential activities to enhance and support lifestyle risk factor modification services to improve referral processes and promote their services.
